Minnesota MDH publishes every licensing survey and complaint investigation for open assisted living facilities, with the full finding narrative and the state's level and scope for each correction order. It removes complaint investigations after seven years when substantiated, four when inconclusive and three when unsubstantiated, and removes all results once a facility closes.
